All Required forms to successfully complete online athletic registration will be completed by selecting large icon above.  This includes:

 1) Read and Sign the WIAA High School Athletic Eligibility Information Bulletin/Signs and Symptoms of Concussion
 2) Complete Athletic Registration Form
 3) Read and Sign the Athletic Code of Conduct
 4) Complete and upload the WIAA Physical Card (Doctor's signature required) or Alternate Year Physical Card.

ALL FORMS MUST BE COMPLETED PRIOR TO THE START DATE OF THE SELECTED SPORT TO ASSURE ELIGIBILITY TO TRYOUT OR PRACTICE ON THE FIRST DAY.

Physical Examination

Students participating in athletics must have a physical examination dated after April 1 of the current year on file with the high school athletic trainer or the junior high nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the sport-specific coach no later than 3 school days prior to the first day of practice/tryouts. Coaches will turn physicals into the trainer or nurse after collection. Physicals may also be uploaded during the online registration process
